Parent/Guardian Checklist for Registration
- Proof of the child's age
Any one of the following constitutes acceptable documentation: birth certificate; notarized copy of the birth certificate; baptismal certificate; copy of the record of baptism – notarized or duly certified and showing the date of birth; notarized statement from the parents or another relative indicating the date of birth; a valid passport; a prior school record indicating the date of birth.
- Immunizations required by law
Immunization Records:
4 doses of tetanus* (1 dose on or after the 4th birthday)
4 doses of diphtheria* (1 dose on or after the 4th birthday)
4 doses of polio (1 dose on or after the 4th birthday)
2 doses of measles**
2 doses of rubella (German measles)**
3 doses of hepatitis B
2 doses of varicella (chicken pox) vaccine or history of disease
A Tuberculosis (TB) test with negative reading is required of any new enrollee from foreign countries, homeless, TB endemic area and any situation deemed high risk.
*Usually given as DTP or DTap or DT or Td
**Usually given as MMR
Acceptable documentation includes: either the child’s immunization record, a written statement from the former school district or from a medical office that the required immunizations have been administered, or that a required series is in progress, or verbal assurances from the former school district or a medical office that the required immunizations have been completed, with records to follow. - Proof of residency (2 documents are required) Acceptable documentation includes: a deed, a lease, current utility bill, current credit card bill, property tax bill, vehicle registration, driver’s license, DOT identification card.
In addition, the following items may be requested at the time of registration but are not required for school registration:
- Student’s last report card
- IEP or 504 Documents
- Court/Custody Order or Guardianship paperwork
